How do I start?
To begin you must register in our members section and provide us with basic information about yourself, your general health, and any medications you are currently taking. You will be given payment options to select from and once your account is verified you may request an appointment for a face-to-face consultation with a doctor closest to where you live.
Before you can receive a consultation we will require a faxed copy of your photo I.D. (Current Driver’s License only please) and medical records. Your records should refer to the condition that you wish to discuss with the doctor and should be as comprehensive as possible. For example: a physical examination made face to face with a qualified physician within the last 12 months must be submitted along with items such as an MRI to back up joint or back problems. This will make it much easier for the doctor to diagnose and prescribe appropriately.
What's the minimum age?
You must be at least 21 years of age to order any services from our site.
How does the consultation work?
Once you have requested a consultation a scheduled appointment will be e-mailed to you. You will be required to attend the doctor’s offices at the appointed time. Please allow plenty of traveling time and aim to arrive 15 minutes before your appointment to allow for checking in etc. Once the consultation is complete the physician will provide you with a plan of care and any prescription(s) for any medically necessary medications, where appropriate, to you for you to take the pharmacy of your choice. In some States the doctor may retain the prescription for “calling in” to your pharmacy.
What does it cost and how can I pay?
We charge a fee for our coordination services to you and then charge a fee on behalf of the doctor for the consultation. The total combined fee for an initial face-to-face consultation is $299. Follow-up face-to-face consultations, normally every 3 months, will cost $209. If a monthly telephone consultation is required in some States this will be charged at $129. You may pay by credit card for these fees, in which case a third party processor will charge your card on behalf of F2FDOC and the doctor. The name that appears on your statement will be “F2FDOC” or “Elmfield”. You may also pay by a money order which should be made payable to F2FDOC. Do I have to use F2FDOC to have any prescriptions I receive filled?
No! At F2FDOC we are your care coordinators, we do not sell or facilitate the sale of prescription drugs, but facilitate medical services and manage your care. We are committed to ensuring that you have the freedom to have any prescription you may receive filled at a pharmacy of your choice, and any rehabilitation or tests at any provider of your choice at a time and price that suits you in the event your physician issues you a prescription or prescribes a treatment. If you receive a prescription, you will receive your prescription directly from the prescribing doctor on each occasion.
Is my information safe and private?
Of course. We ensure your complete confidentiality at all times and we do not disclose any details to anyone except your physician. You and he/she will establish and maintain a valid physician/patient relationship and both F2FDOC and he/she will keep your medical history information in a patient file in a secure place.
Is my payment information secure?
Our on-line ordering system uses the latest in Secure Technology (SSL128BIT). All personal and credit card information (where applicable) is submitted with the highest level of security and care. Our systems also include safeguards to protect against credit card fraud. Individuals attempting the fraudulent use of a credit card or money order will be reported to the appropriate authorities.
Is This Legal?
We are always committed to meeting and exceeding standards. Our mission is to ensure patients receive care for conditions by appropriate physicians, particularly pain management. By participating in ongoing face-to-face consultations with a physician under a physician’s plan of care you are establishing a valid doctor/patient relationship as you should. We introduce only US-licensed physicians to our patients.

In addition to your payment, before we can schedule a consultation we will require a faxed copy of your latest medical records which support your pre-existing condition, a copy of the last physical examination you received from a licensed and qualified physician and a copy of your photo ID (Current Drivers License) please.
